Founded in 1961,the Louisiana Association for Challenged Adults (LAC) has been a cornerstone of the Walker community, providing essential services to adults with intellectual and developmental disabilities. Our goal is to help participants lead fulfilling and independent lives through personalized support, community integration, and care.

NOTICE OF PRIVACY PRACTICES Effective Date: February 16, 2026 THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. OUR PLEDGE REGARDING YOUR PROTECTED HEALTH INFORMATION (PHI) We understand that information about you and your health is personal. We are committed to protecting that information. This Notice describes the privacy practices of Louisiana Association for Challenged Adults and how we handle your information to provide Day Habilitation and Supported Employment services. HOW WE USE AND DISCLOSE YOUR INFORMATION State Mandated Reporting: As a provider for the Louisiana Department of Health (LDH), we are required by state law to report all critical incidents to your Support Coordinator (SC). We provide a report to your SC within 24 hours of discovery. Your SC then enters this information into the Statewide Incident Management System (SIMS) as required by the Office for Citizens with Developmental Disabilities (OCDD). Substance Use Disorder (SUD) Protections: If we receive or maintain records regarding SUD treatment, these records are protected under 42 CFR Part 2. We will not disclose these records in civil, criminal, or administrative proceedings without your specific written consent or a court order. Redisclosure: Once we disclose your PHI to a third party based on your authorization, that information may be reshared by the recipient and may no longer be protected by federal privacy laws. YOUR RIGHTS REGARDING YOUR PHI •Right to Inspect and Copy: You have the right to inspect your records. Under 2026 federal guidelines, you have the right to take photos of your records using your own device at no charge during a scheduled inspection. •Right to Opt-Out: You have the right to opt-out of any fundraising communications or marketing outreach we may conduct. •Right to File a Complaint: If you believe your privacy rights have been violated, or if you have concerns regarding the quality of our facility or services, you may file a complaint with: 1.Our Privacy Officer: 225-664-7384 or director@lac-la.org. 2.LDH Health Standards Section (HSS): Call (225) 342-0138 or email HSSComplaints@la.gov. 3.LDH Privacy Office: Call (225) 342-3855 or email Privacy-BHSF@la.gov. 4.Federal Government: Contact the U.S. Dept. of Health and Human Services (OCR).
